service agreement

This form is provided to all clients

I _____________ (“client”) agree to 45 minute Success Life Skills sessions with Athena St.Juste, MHC, with no minimum or maximum requirement. Client acknowledges and agrees as follows: • All information discussed during sessions will be kept confidential. • The Life Coach must report any verbalized wish by client to harm self or others. • Client and Life Coach will meet virtually, or in person, as needed on an agreed regular basis. Client will make a Venmo, Paypal or check payment prior to the start of our session. • Each virtual session is $80.00. In person sessions are on a sliding scale ($80-100). Client understands that payment will be due prior to the scheduled session time. Cancellations must be called in 24 hours in advance.
